All new patients need to complete a Registration Form. To help speed up the registration process, please print off and complete the Registration form by clicking here Registration Form and bring this with you when you come to the Surgery together with the following evidence.
ID Evidence needed for Registration
To register at the surgery, please print off and complete the Registration form (which you can find by clicking on the tab above that says Registration Form) and bring it with you to the Surgery together with proof of your ID AND also your current address. You must bring one original document from each of the two lists below.
LIST 1: Proof of ID (must contain a photograph and be current):
- Driving Licence
- NYCC parking card for disabled people
- Citizen Card
- Bus Pass
LIST 2: Proof of Address (must be dated within the last three months)
- Bank of Building Society statement
- Gas / electricity bill
- Telephone bill
- Council tax bill
If you do not have this evidence then please contact us.
If you are registering a child under the age of 16 then please bring evidence of your link to the child e.g. a birth certificate or a letter of adoption.
The Surgery is usually very busy between 8am and 11am, and 2pm and 6pm when the doctors and nurses are seeing patients. It would be much appreciated if you are coming to register if you could come between 11am and 2pm when the Surgery is slightly quieter. If you are not able to come during these times then please be prepared to wait. Thank you in advance for your patience.
